What is SASH?
Created by Cathedral Square, Support and Services at Home (SASH) programming utilizes congregate housing to provide targeted support and in-home services to aging individuals and individuals with disabilities to enable individuals to age healthfully, independently, and with dignity in their homes and communities.
SASH Program and Eligibility Details
Participation in SASH’s year round service is voluntary, free of charge, and open to eligible residents of affordable housing sites age 65 and older and / or with disabilities, as well as Medicare recipients in surrounding communities. Administration of the program is via a contracted SASH coordinator who connects participants with resources from social-service agencies and community health providers.
Programs include comprehensive health and wellness assessments, creation of individualized care plans, one-on-one nurse coaching, care coordination, and health and wellness group programs such as “Bone Builders,” smoking cessation, and Tai Chi programs.
SASH in Lamoille County and Hardwick
Lamoille Housing Partnership’s SASH coordinator serves more than 70 participants residing in Cambridge, Hardwick, Hyde Park, Jeffersonville, Johnson, Morrisville, Stowe and Waterville.
Our SASH sites include Maple Street Apartments in Hardwick and Lamoille View Apartments in Morrisville.

SASH Benefits and Impact
According to a third party evaluation of older adults living in Vermont affordable housing sites enrolled in SASH, the benefits include improved health and quality of life, slowed and decreased Medicare expenditures, more health-savvy older adults, a reduction of $1,200 in average individual Medicare expenditures reflected across hospital, emergency room and specialist visits; the largest cost savings were for participants eligible for Medicare and Medicaid.
The success of participants reflects the numerous benefits of the nationally recognized program, ranging from improving ability to manage medications and increasing overall functional status, to reducing social isolation and smoothing transitions to nursing home care.
